Introduction: The human immunodeficiency virus (HIV) can lead to a loss of lymphoid tissue through the intestinal mucosa, making the intestine susceptible to infections and inflammation. As a result, the patient may present with Crohn's Disease (CD), a chronic inflammatory bowel disease (IBD). Short Bowel Syndrome (SBS) results from loss of bowel absorption capacity after extensive bowel resection and is associated with several complications. Objective: To report the case of a patient presenting with acute chronic diarrhea, of probable multifactorial etiology, secondary to HIV, CD, and SBS. Methods: The information contained in this report was obtained through a review of the medical record and a review of the literature. Case Report: This is the report of E. F. C, male, 49 years old, who was admitted to the ICU of Hospital Ernesto Dornelles, in Porto Alegre, from 12/04/2020 to 01/07/2021, with HIV, CD, and SBS. The patient was admitted complaining of diarrhea that had started 15 days ago, which had worsened in the last 7 days, with loss of appetite, vomiting episodes, and mental confusion. Using empirical Lamivudine, Zidovudine, Tenofovir, Ciprofloxacin and Metronidazole, Methotrexate and Prednisone. On physical examination, he was sleepy, afebrile, with a normotensive abdomen, severe dehydration, and edema 3+/4+. The complexity of sleep hinders the formulation of sleep guidelines. Recent studies suggest that different unhealthy sleep characteristics jointly increase the risks for cardiovascular disease (CVD). This study aimed to estimate the differences in CVD-free life expectancy between people with different sleep profiles. We included 308,683 middle-aged adults from the UK Biobank among whom 140,181 had primary care data linkage. We used an established composite sleep score comprising self-reported chronotype, duration, insomnia complaints, snoring, and daytime sleepiness to derive three sleep categories: poor, intermediate, and healthy. We also identified three clinical sleep disorders captured by primary care and inpatient records within 2 years before enrollment in the cohort: insomnia, sleep-related breathing disorders, and other sleep disorders. We estimated sex-specific CVD-free life expectancy with three-state Markov models conditioning on survival at age 40 across different sleep profiles and clinical disorders. We observed a gradual loss in CVD-free life expectancy toward poor sleep such as, compared with healthy sleepers, poor sleepers lost 1.80 [95% CI 0.96–2.75] and 2.31 [1.46–3.29] CVD-free years in females and males, respectively, while intermediate sleepers lost 0.48 [0.41–0.55] and 0.55 [0.49–0.61] years. Mutations in mitochondrial energy-producing genes lead to a heterogeneous group of untreatable disorders known as primary mitochondrial diseases (MD). Leigh syndrome (LS) is the most common pediatric MD and is characterized by progressive neuromuscular affectation and premature death. Here, we show that daily cannabidiol (CBD) administration significantly extends lifespan and ameliorates pathology in two LS mouse models, and improves cellular function in fibroblasts from LS patients. CBD delays motor decline and neurodegenerative signs, improves social deficits and breathing abnormalities, decreases thermally induced seizures, and improves neuropathology in affected brain regions. Mechanistically, we identify peroxisome proliferator-activated receptor gamma (PPARγ) as a key nuclear receptor mediating CBD’s beneficial effects, while also providing proof of dysregulated PPARγ expression and activity as a common feature in both mouse neurons and fibroblasts from LS patients. Taken together, our results provide the first evidence for CBD as a potential treatment for LS. Mutations in mitochondrial genes cause untreatable diseases such as Leigh syndrome (LS). Here, authors show that cannabidiol (CBD) administration can extend lifespan and improves pathology in LS mouse models, mediated by PPARγ.
